A 69-year-old delivery driver exhibited pronounced skin aging, including deep wrinkles and thickened texture, predominantly on the left side of his face. This was linked to the 28 years he spent driving with the driver-side window closed, placing that side closest to the glass most of the time.
This remarkable case was detailed in the New England Journal of Medicine. Physicians diagnosed the condition as unilateral dermatoheliosis, which is skin damage caused by asymmetrical ultraviolet light exposure. It illustrates that UVA rays can penetrate car windows and cause skin damage progressively, without causing immediate sunburn.
The man noticed a “gradual thickening and wrinkling” on the left side of his face developing over nearly 25 years. Although symptom-free, the aging discrepancy between both facial sides became increasingly obvious as damage accumulated.
Unequal Sun Exposure Drives Asymmetric Facial Aging
Examination revealed noticeable wrinkles, skin thickening, blackheads, and small nodules on the sun-exposed side. Microscopic tissue analysis showed alterations to the dermis layer, including degenerated elastic fibers and small cysts around fine hair follicles.
The characteristics matched Favre-Racouchot syndrome, a condition linked to severe sun-induced skin deterioration, marked by rough wrinkles, yellowish thickening, and follicular changes. In this case, these features appeared mostly on the side close to the truck window.

Chronic UVA exposure thickens the epidermis and stratum corneum, the dead cell layer protecting the body. It also damages skin’s elastic fibers, reducing elasticity and causing roughness and deeper wrinkles over time.
This driver’s face provided a distinct firsthand example since other aging factors like genetics and lifestyle were identical across both sides. The main variable was the amount of sunlight each side endured during work.
UV Damage Affects More Than Just Wrinkles
While the primary concern was visible skin aging, ultraviolet radiation also harms cells beneath the surface. UVA contributes to DNA and cellular damage, and UVB is strongly linked to mutations causing skin cancer.
Though no skin cancer was detected, doctors advised ongoing skin checks given the patient's extended sun exposure. They also recommended sun protection and the use of topical retinoids, vitamin A-based creams that can help improve thickened, damaged skin.
This case doesn’t imply that every driver will develop such pronounced uneven facial damage, but it highlights how long-term, low-level UV exposure accumulates, especially for those frequently beside vehicle windows—truckers, delivery agents, taxi drivers, and more.
Comprehensive protection should target both UVB and UVA rays. Broad-spectrum sunscreens, protective apparel, hats, shade, and window filters help reduce UV radiation affecting the skin.
Occupational Sun Exposure Can Have Significant Long-term Effects
While the striking photo emphasized visible contrast, the clinical significance lies in the gradual accumulation over years. The damage was not the result of a single intense incident but built up from consistent, repeated exposure through the window glass during daily driving.
This case underscores the importance of considering occupational sun exposure even for indoor or enclosed environments like vehicle cabins. Exposure to ultraviolet radiation can be substantial without outdoor sun or direct head exposure.
